Form fields marked with an asterisk (*) are required.
In order to be placed on the ballot, this form must be completed by April 8th at noon.  Students can only run for 1 position.  In order to be placed on the ballot, candidates must attend one of the Candidate Training sessions. If you do not attend session, you may still run as a write-in candidate.  See neiu.edu/sld Student Government tab - elections tab for more information and for dates of trainings

AGREEMENT TO SERVE & TRAIN
If elected, I will serve in my position for the entire elected term. This includes my attending the mandatory SGA day retreat in May as well as Summer Retreat scheduled as an off campus, over-night event that will take place August.

CANDIDATE CERTIFICATION:
My participation in the forthcoming election will be undertaken in accordance with and will be governed by: all applicable University policies and campus regulations; the Student Government Association Constitution and By Laws; and the Student Government Election Code, as interpreted and implemented by the Election Commission. I acknowledge that I may receive sanctions and/or be disqualified from the election process upon infraction of the Election Code. I acknowledge I am responsible for reading, understanding and abiding by all election regulations.
